Partner Visa Forms

Most of these forms are completed inside ImmiAccount rather than downloaded. The ones that are still PDFs are linked below from the Department's own form listing. Form numbers, titles and edition (design) dates were checked against the PDFs on immi.homeaffairs.gov.au on 23 September 2026.

Every partner visa form, and who completes it

Forms 1005 and 1006 belong to the bridging visa, not the partner visa itself. Travel while a subclass 820 application is open is covered on this site's travel page.

Which partner visa forms are mandatory, and which are only on request?

1. Always needed: the applicant's online application, the sponsor's online sponsorship (Form 40SP), the supporting witness statements (usually Form 888), and at the permanent stage the online "Stage 2" form. Home Affairs also lists Form 80 with the partner applicant's character documents.
2. Only when something applies: Forms 47A and 1229 (children), 815 (a health undertaking the Department asks for), 1221 (on request), 956 and 956A (appointments), 1022 and 1023 (changes and errors), 1436 (adding a child), 1446 (withdrawal), the Partner Processing Enquiry Form, and Forms 1005 and 1006 (bridging visas).

Form 47SP, the applicant's partner visa form

Form 47SP was the paper application for migration to Australia by a partner. As at 23 September 2026 the Department no longer publishes it: the PDF address returns a not-found page, and the Partner Visa FAQs say you must apply online, with paper applications accepted only by invitation. The application questions are now answered in ImmiAccount under "Stage 1 – Partner or Prospective Marriage Visa", and there is no current paper edition to download.

Form 40SP, the sponsor's partner visa form

• The Department calls it "Sponsorship for a Partner to Migrate to Australia".
• Only the sponsor completes it, online, after the applicant has lodged.
• It is made with the applicant's TRN or Application ID, from either partner's ImmiAccount.
• It asks for proof of the sponsor's status and police certificates, plus written consent to disclose relevant convictions to the applicant.

Form 888, the partner visa supporting statement

Form 888, "Supporting statement in relation to a Partner or Prospective Marriage visa application", is completed by someone aged 18 or over who knows you both. The current edition is dated August 2026. Home Affairs asks for statements from two witnesses, and the form says a witness may be asked for up to three statements while the application is processed. Applicants in Australia without a substantive visa need Commonwealth statutory declarations instead, because the Department says Form 888 is not a statutory declaration. Form 80, the 10-year personal history for a partner visa

Form 80 is a 19-page PDF for applicants aged 16 or over. It asks for 10 years of addresses and international travel, your full employment history with no gaps, and a long list of character questions. The Home Affairs subclass 820 and 309 pages tell partner applicants to complete it and attach it with their character documents. The Form 80 page walks through it.

Forms you use after you lodge a partner visa

• Form 1022 reports a change in circumstances. Section 104 of the Migration Act 1958 (Cth) requires you to tell the Department in writing, as soon as practicable, whenever an answer in your application stops being correct.
• Form 1023 corrects an answer that was wrong when you gave it, as s 105 requires.
• The Partner Processing Enquiry Form is for the situations the Department lists, such as asking for more time, adding a child, having no access to ImmiAccount, or withdrawing a sponsorship. It is set out on the page about the partner visa processing enquiry form.

Where do you download partner visa forms?

Download them only from the Department. Every PDF in the table links to immi.homeaffairs.gov.au, and the full list is on the Department's PDF forms page. Form-hosting websites often keep old editions. The edition date is printed at the foot of every page, for example "888 (Design date 08/26)". Check it against the Department's copy before you sign.

Are partner visa forms available in Word or as editable PDFs?

Not in Word. The Department's PDFs are fillable: each one says to open it in Adobe Acrobat Reader and type in the fields, or print it and write in English in block letters. Sign and date it where the form asks, then scan it in colour and attach it in ImmiAccount.

What is MOD P, and is it a partner visa form?

No. MOD P is a Services Australia form for a partner's details in a Centrelink claim, and it has nothing to do with a Home Affairs partner visa application; the site's glossary explains the difference.

Questions about partner visa forms

Do you have to print and sign the forms?

The online application and sponsorship are declared on screen. PDF forms such as 888, 80, 1022 and 1229 carry a signature line. Sign them, scan them and attach them.

What happens if you use an old edition?

The Department's current edition is the one it publishes. Download each form fresh from immi.homeaffairs.gov.au so that the design date matches.

Can the sponsor complete the applicant's form?

The sponsor may help with clerical work, and s 280(5C) of the Migration Act 1958 (Cth) lets a sponsor give immigration assistance to the applicant. The declarations must still be made by the person they belong to.

How do you fix a mistake after lodging?

Use "Notification of incorrect answer(s)" under Update details in ImmiAccount, which is the online Form 1023.

Do the forms change on 1 July?

Not on a set date. Visa charges can change on 1 July, as the Pacific and Timor-Leste concession did in 2026, but a form changes whenever the Department reissues it. Form 888, for example, was reissued in August 2026.

Getting the forms right

A missing form or an old edition can hold up a partner visa or, for some applicants, make it invalid. A registered migration agent or an Australian legal practitioner knows which forms your circumstances need and checks each one before it is attached. That makes an application they prepare more likely to succeed, though no one can promise a grant.

Questions people ask

Is there a PDF application form for a partner visa?

No. As at 23 September 2026 the Department does not publish Form 47SP or Form 40SP as PDFs; both links on its form listing return a not-found page. The applicant applies online in ImmiAccount and the sponsor lodges the sponsorship online. A paper application is accepted only by invitation.

Which partner visa forms are mandatory?

The applicant's online application and the sponsor's online sponsorship are always needed. Home Affairs also asks for supporting witness statements, usually on Form 888, and lists Form 80 with the character documents. The other forms apply only when a child is included, a change or error must be reported, or the Department asks.

Are partner visa forms available in Word?

No. The Department publishes its partner visa forms as PDFs that can be typed into using Adobe Acrobat Reader or printed and filled in by hand in block letters. It does not publish Word versions, and sites that offer editable copies may serve an old edition.

Is MOD P a partner visa form?

No. MOD P is a Services Australia form about a partner's details for Centrelink payments. It plays no part in a Department of Home Affairs partner visa application.
